Measure What Matters

Konnected Impact™ is KTI’s social impact intelligence layer, turning SDOH intelligence platform into a customized Social Impact Index for your organization. Powered by our patent-pending LME system, it captures 24/7 continuous resident feedback and analyzes 320 social determinants of health indicators across five domains: Economic Stability, Education Access, Healthcare Access, Neighborhood, and Social & Community Context.

Unlike static one-time assessments, Konnected Impact™ monitors continuously, using predictive analytics to identify at-risk residents before crisis, alerting social workers in real time, and generating automated Medicaid-billable documentation. The result: evidence-based impact reports that secure funding, drive policy, and keep residents housed.

Now in pilot deployment, partner access available 2026.

Resident data changed city policy. That winter.

Denver’s unhoused population was evaluated on cold-weather preparedness using KTI’s LME system. The data went directly to city council — and they acted. Resident voices, turned into data, turned into policy.

“The report was instrumental in passing the decision to expand shelter services.”

32°F → 6°F

Shelter activation temperature lowered — keeping Denver’s most vulnerable residents safe on nights the old threshold would have left them outside.

From Data to Action, City of Littleton

KTI partnered with the City of Littleton and a senior residential community to create a pathway to permanent housing and ongoing health supports for residents living in housing instability. The LME system gave residents a safe, continuous way to share what they were experiencing and that data went directly to social workers and city leadership. The data exposed current needs and established a health baseline. Over the next year KTI continued following up with residents, LME amplifying voices and sending boots-on-the-ground alerts to social workers so they could get residents what they needed, when they needed it.

Resident voices that automatically trigger Medicaid billing.

KTI built a Universal Medicaid API linked directly to resident voices. When a resident says “I’m worried about paying rent” or “I skipped my medication this week,” the platform auto-triggers the appropriate SDOH Medicaid billing codes — for any Medicaid service, not just housing. Automatic SDOH API Medicaid.

KTI was selected as a Medicaid pilot finalist — one of eight companies globally — and FHIR API integration is in development for direct EHR connectivity.
